Overview

Tirzepatide is a 39-amino acid synthetic peptide that functions as a dual agonist at both the glucose-dependent insulinotropic polypeptide (GIP) receptor and the glucagon-like peptide-1 (GLP-1) receptor. It was designed to combine the complementary metabolic actions of two distinct incretin hormones into a single molecule. Unlike monoagonist GLP-1 receptor agonists, tirzepatide's simultaneous engagement of the GIP receptor contributes additional and synergistic effects on glucose homeostasis, adipose tissue biology, and energy balance in research models.

Mechanism

Tirzepatide binds to and activates both the GIP receptor (GIPR) and the GLP-1 receptor (GLP-1R) with high affinity. GIP receptor activation stimulates glucose-dependent insulin secretion, suppresses glucagon, and has been shown in preclinical models to directly influence adipocyte lipid metabolism and reduce adipose tissue inflammation. GLP-1 receptor activation independently stimulates insulin secretion, suppresses glucagon, slows gastric emptying, and engages central appetite-regulating pathways in the hypothalamus and brainstem. The dual mechanism produces additive or synergistic effects on insulin sensitivity and energy expenditure compared to GLP-1 receptor agonism alone.
